  • Counselor,
  • Relationship,
  • How is the person related to the employee, and
  • Referral Type, Source and Reason.

    The data grid will display all the patients for this specific case, and the manager will be notified if the referral was a Formal Referral

  • EAP Completed
  • EAP not completed. Schedule an appointment (see below)
